EcoQuest International has been investigating...
 ...the
potential of making health insurance and financial services
available to EcoQuest dealers. EcoQuest International is now at a
critical stage in its process, and needs your help!
Please complete the health insurance survey via the direct link to
EcoQuest International at the end of this email.
In the survey, there are questions regarding your personal health
information, which EcoQuest International is asking you to answer.
This is not an application for health coverage; however it is
critical that you answer these questions in order for staff to
proceed with obtaining preliminary proposals of coverage from health
insurers. It will be virtually impossible to obtain a viable health
program without the health data.
EcoQuest International is in full compliance with the Health
Insurance Portability and Accountability Act ("HIPAA") and other
privacy laws. Only the minimum necessary data will be utilized for
this process. Personal identifiers such as your name, dealer# and
address will be removed prior to distribution to the carriers.
EcoQuest International maintains physical, electronic and procedural
safeguards that comply with Federal regulations to guard your
nonpublic information. Once the project is completed, all personal
health information they have collected will be destroyed.
As a dealer, I do not have access to your protected information that
you submit in the survey.
